Options for Accessing Delirium Data

There are multiple options for accessing delirium program data. The following information will help you find everything from extensive raw data that can be used in research to pre-formatted canned reports for general operations or ongoing quality improvement efforts.

Requesting Access to Raw Data for Research or Quality Improvement:

  • Click here for instructions on how to request access to specific raw data points available in the Delirium Reduction Data Set that includes hundreds of data points covering:
    • Patient demographic data
    • Admission data (admission source, date, service, Dx, problem list)
    • Delirium data (screening scores and dates, delirium incidence, delirium prevalence)
    • Discharge data (D/C Dx, Service, Attending, disposition)
    • Other general data (risk of mortality, CMI aLOS, CMI, Readmission information)

Instructions on how to request access:

 

  1. Use this form to submit your request.
    1. Organization: Health
    2. Type of Request: Request New Access
    3. Type of Role: Tableau Viewer Access (no license required)
    4. Check “User requires access to Production”
    5. Select Production project(s) the user needs access to: Enterprise Information and Analytics Limited Access Dashboards

 

  1. Add yourself by searching using your first name first:

  1. Your information will autopopulate. If you have not yet received SVP approval, that is required. Please confer with your manager and select the SVP you roll up to. Select “Add.”

  1. In the last section:
    1. Identify why you need access
    2. Why is this access being requested: New access for team member
    3. I will be using Tableau for the purposes of: (select most appropriate choice)

  1. Finally, add in your manager’s name and submit.
  1. If you have not completed an attestation form or your form is out of date, you will receive an email from Docusign. This form is required prior to receiving access to the dashboard.
  2. The dashboard requires the following criteria to be met. Once this is complete, access will be provisioned within 24 hours:
    1. SVP approval on file
    2. Active attestation on file
    3. Manager approval

Finding and Running formatted Delirium APeX Reports for General Operations or Ongoing Quality Improvement

  • Click here for instructions on how to find and run pre-formatted APeX reports that can be used for Quality Improvement or other general operational purposes:
    • Delirium Reduction - RN Feats of the Month which includes:  
      • AWOL and NuDESC compliance month to month (broken into AM and PM shifts)
      • Total pts and total pts with delirium for month report ran
      • Great saves (# of pts who screened AWOL+ but didn’t develop delirium)
    • Delirium Screening Nursing Compliance by Shift which details:    
      • % compliance with NuDESC screening by nurse by shift  
    • Delirium Reduction Program Summary Report by Unit which generates 12 month run charts of the following data:
      • Delirium-days per 1000 pt days (delirium rate)
      • Safety Attendant Days per 1000 Pt Days (**NOTE—this is different than the financial safety attendant day measure)
      • Delirium Incidence
      • Delirium Prevalence
      • Mean LOS for Delirious Pts
      • Mean LOS for non-Delirious Pts
      • NuDESC Compliance
      • AWOL Compliance

 

 

 

Requesting Access to Raw Data for Research or Quality Improvement

For access to all or some of the Delirium Reduction Program’s data, please complete this request form.  Please have ready: 

1.    Email address and role for primary contact person

2.    General summary of the proposed project (to be entered as free text)

3.    Funding source(s), including Grant type and number/period (if applicable)

4.    IRB/CHR approval or exemption number and dates (if applicable)

5.    Research rationale (i.e., 3-page max document ready to be uploaded), including:

      a.    Purpose

      b.    Background/Significance

      c.    Preliminary data (if applicable)

      d.    Methods and analysis plan

Finding and Running formatted Delirium APeX Reports for General Operations or Ongoing Quality Improvement

 

 

1. Log into APeX

2. Click on the Epic                      dropdown                             

a) Click on Reports               

b) Select My Reports 

 

 

 

 

3. Click on Library

4. Enter the name or number of  the report you want to find in the Search the Library field

5. Click on New Report

 

 

 

 

6. In the Criteria tab, enter    appropriate report  parameters  (**these will vary by report)

 

7. Click on General Tab

a) Enter a Name for the report (something meaningful to you)

b) Click Save

NOTE: these steps allow you to save your own version of the report using the report template; you won’t be updating master report

c) Click Close 

 

8. The report you just created will appear beneath the report template header

9. Click on the STAR next to the report so that it will appear in your My Reports tab

 


RE-RUNNING YOUR REPORT IN THE FUTURE (tip to avoid all prior steps just to run the same report for a different timeframe or department)                                                         

Go into My Reports:

  • Right click on the report you want to update
  • select Edit
  • update parameters and click Save
  • right click on report and select Request Report

8. The report you just created will appear beneath the report template header

9. Click on the STAR next to the report so that it will appear in your My Reports tab


10. Click on My Reports Tab

  1. Right Click on report
  2. Select Request Report
